What this is based on
The examples below come from the public evidence pack, including both raw answer keys and the completed source-claim map. The failure taxonomy is a review tool. It is not a claim that every listed failure appeared in the captured runs.
The quiz is not the risky part
An AI-generated quiz looks disposable. If a question is awkward, you skip it. If two questions repeat the same idea, you roll your eyes and move on. The answer key feels different. It looks like the settled layer, the part that tells you whether your memory is correct.
That is why the key deserves more scrutiny than the questions.
A weak question wastes a minute. A fluent wrong answer can become the standard you use to correct yourself. On the next review, you are no longer comparing your recall with the PDF. You are comparing it with a sentence that merely sounds finished.
The danger is not limited to spectacular hallucinations. The more common operational risk is smaller: a definition loses a condition, an example becomes a fact, a partially correct answer is marked complete, or the key answers a neighboring question. Those errors are easy to absorb because answer-key prose is short, direct, and presented without visible uncertainty.
This matters especially in a retrieval workflow. The synthetic handout used in this site's same-source test defines retrieval practice as trying to recall information before looking at the answer. Its example then tells the learner to compare the response with the source and repair mistakes. Replace the source with an unchecked AI key, and the repair loop can point in the wrong direction. That last sentence is a workflow inference, not a measured learning result from our test. We did not test retention or exam scores.
The practical rule is simple: generate questions quickly, but promote answers to trusted status slowly.
What the local test actually showed
The evidence here is deliberately narrow. We used one synthetic, one-page PDF-style handout about attention, memory, retrieval practice, spacing, recognition, and recall. We submitted one shared task and captured one NotebookLM run and one ChatGPT run. The source was supplied as source-equivalent copied text because local file-picker automation was unreliable. Neither output was regenerated. No real students, retention intervals, or exam outcomes were tested.
Within that limited test, both tools produced ten-question quizzes with answer keys. The local claim map found the answer-key items aligned with the source. NotebookLM's first five answers correctly identified attention, working memory, long-term memory, rereading without retrieval, and spacing. Its later answers also matched the handout, including the instruction to compare a recalled photosynthesis answer against the source and repair mistakes. ChatGPT's key correctly described attention, working memory, long-term memory, retrieval practice, spacing, recognition versus recall, and transfer questions.
That result matters because it keeps this column honest: our captured run did not reveal a wrong answer key. I am not going to manufacture a failure to make the headline more dramatic.
The test did reveal conditions that change how safely a key can be checked. NotebookLM preserved citation chips and visible source affordances. ChatGPT's visible answer key did not include source citations, even though its answers followed the supplied material. The scoring sheet therefore gave NotebookLM 4 out of 5 for student safety and ChatGPT 3 out of 5. That was a workflow judgment about verification affordances, not proof that one model is universally more accurate.
The unsupported-claims log recorded one minor ChatGPT extrapolation: outside the quiz key, it explained recognition with an answer-sheet scenario that was consistent with the source definitions but not directly stated in the handout. That example was logged as partial or explanatory, not as a major educational error. It demonstrates the boundary an audit needs to preserve. A useful explanation can still be something the source never said.
Both tools also handled an over-demanding part of the prompt well. Asked for three concepts students were likely to confuse, NotebookLM named recognition and recall, then marked the third slot Not in source. ChatGPT named the same supported pair and marked slots two and three Not in source. The source identified only that one confusion. A safe key needs this same willingness to leave a slot empty rather than complete the pattern.
So the evidence does not say, "AI answer keys are usually wrong." It supports a narrower conclusion: answer keys are high-leverage outputs, and the ability to verify them varies even when the answers happen to be correct.
Why fluent errors are sticky in a study loop
An answer key occupies three roles at once.
First, it is a grading rule. It decides whether your response counts as correct. Second, it is a replacement response. When you miss, the key supplies the wording you are likely to rehearse next. Third, it is a compression layer. A paragraph from the source becomes one or two sentences, often without the qualifications that made the original accurate.
That combination gives a small error reach. Imagine a source says, "Recall is producing the answer without seeing it first." A key that says, "Recall is recognizing the correct answer" would not merely misdefine a term. It would certify the exact confusion the handout warns against. This is an illustrative counterexample, not an error observed in either captured output.
Fluency compounds the problem. A fragment or visibly uncertain answer invites checking. A polished answer with parallel formatting, confident punctuation, and ten neighboring correct answers does not. The learner may audit the difficult question and trust the easy-looking definition, even though definitions are where one missing word can reverse a contrast.
The right design response is not to ban generated quizzes. It is to separate generation from authorization. Let the model draft the key. Let the source approve it.
The failure modes worth checking
These categories are an audit taxonomy, not a count of failures found in the one-run test.
Unsupported addition
The answer includes a fact, cause, example, or condition absent from the source. The addition may be true in the wider world and still be wrong for a source-bounded assignment.
The local ChatGPT answer-sheet example shows the mild version. It was compatible with the handout, but it was an explanatory addition. If that kind of material appears in a key, label it generated explanation rather than letting it masquerade as source wording.
Omitted qualifier
The answer preserves the topic but drops the limiting phrase. "Practice exams should test recall" is close to the handout, but the source says they should test recall, "not only recognition." That contrast is the instructional point. A key can look correct while deleting what the question was meant to test.
Neighbor-answer substitution
The response is accurate about the chapter but answers a nearby concept. Attention, working memory, and long-term memory all appeared together in the test handout. A fluent description of working memory would still be wrong if the question asks which process selects information for deeper processing.
Granularity mismatch
The key may be too broad or too narrow for the prompt. "Study over time" gestures toward spacing, but it may not be enough if the question asks for the three steps of spaced retrieval: recall, wait, and recall later. Conversely, a key can require details the source never makes necessary, causing a valid student response to look incomplete.
Example-to-rule promotion
A source example becomes a universal definition. The handout used photosynthesis to demonstrate closing notes, answering from memory, comparing with the source, and repairing mistakes. Photosynthesis is an instance of the method, not part of the definition of retrieval practice.
False completeness
The prompt asks for three, five, or ten items, and the model fills every slot despite thin source support. The observed runs avoided this failure in the confusing-concepts section by writing Not in source. An answer-key audit should reward that behavior, not punish the model for returning a shorter list.
Question-key disagreement
The question changes during generation, but the key answers an earlier draft or a different interpretation. This is not a sourcing problem alone. You must compare three objects: question, answer, and source. Checking only whether the answer sentence appears in the PDF can miss the fact that it does not answer the question asked.
Verification theater
A citation marker, page number, or source label creates confidence without establishing entailment. NotebookLM's source affordances made checking easier in our run, which is useful. They did not transfer authority away from the original handout. A citation is a route to the evidence, not the verdict.
A seven-pass answer-key audit
Use the full pass for graded or professional material. A shorter quiz still needs every answer checked.
- Freeze the packet. Save the exact questions and key before editing. Otherwise you can accidentally repair a question while leaving evidence tied to the old version.
- Atomize each answer. Split compound answers into separate claims. "Retrieval is harder and always improves memory" contains at least two claims and should not receive one blanket check.
- Match question to answer. Ask whether the key responds to the wording, scope, and requested number of parts. Do this before opening the source.
- Trace every claim. Record the section, page, quote, or source cue that supports it. For the local handout, the answer "working memory" traces to the core-ideas sentence describing a temporary workspace.
- Assign a status. Use
supported,partly supported,not supported, orquestion ambiguous. Do not collapse partial support into a pass. - Repair both sides. If the source supports a narrower answer, rewrite the answer and possibly the question. If the source supports nothing, remove the item. Do not ask the model to improvise a replacement from memory.
- Retest blind. Hide the audited key, answer again, then compare with the approved version. This preserves the source handout's sequence: recall first, check second, repair third.
Use a small ledger while auditing:

The status column is more important than a total score. One unsupported definition can matter more than three stylistic problems.
Copy the answer-key audit prompt
Run this after the quiz has been generated, with the original source still available:
Audit the answer key against the provided source only.
Do not use outside knowledge.
For every quiz item, return a table with:
1. question number,
2. the exact answer-key claim,
3. whether the answer directly answers the question,
4. source quote or precise source cue,
5. status: Supported, Partly supported, Not supported, or Ambiguous question,
6. the smallest safe correction.
Rules:
- Split compound answers into atomic claims.
- Preserve qualifiers, conditions, numbers, and contrasts.
- Label generated examples as Generated example.
- If the source does not contain enough information, write Not in source.
- Do not invent a replacement fact.
- If the question itself is defective, rewrite or remove the question.
- End with a clean answer key containing only audited answers.
Then apply the human checklist:

Limits
The same-source run offers a useful positive example, not a failure benchmark. Both generated keys were source-aligned in the local claim map. NotebookLM made the source trail more visible. ChatGPT produced a correct, compact key but required more manual source tracing in the captured display. Both tools showed good refusal behavior when the prompt requested more confusing concepts than the handout supported.
That is enough to justify an audit habit. It is not enough to estimate an error rate, rank current models broadly, or claim that students learned more with either tool. The test covered one synthetic handout and one run per tool. Tool behavior, source complexity, extraction quality, and prompt design can all change the result.

Treat the answer key as executable study logic. Questions tell you what to attempt. The key tells you what to install as the correction. Draft it with AI if that saves time, but do not let polished formatting sign its own approval. The original source should get the final vote.
